Buttock Implants
As a board-certified plastic
surgeon practicing in Manhattan I have witnessed a dramatic rise in the
number of patients in the New York metropolitan area requesting an
improvement in their buttocks. I now perform more buttock augmentations
using buttock implants than any other board-certified plastic surgeon in
the New York area. My first encounter with the concept of buttock
augmentation was during a trip to Peru in the spring of 1997. I was
participating in an international plastic surgery exchange focusing on
aesthetic and reconstructive surgery. In South America the aesthetic
ideal is somewhat different than it is here in the U.S. In South America
the buttocks are the focal point of sensuality. I discovered that
plastic surgeons in South America were routinely augmenting the buttocks
in an attempt to meet the demands of a buttock conscience culture. It
was not until years later that this procedure became popular in the U.S.
Buttock augmentation is now routinely performed in the U.S. in only a
few states primarily Florida, California and New York. The influence of
the Latin community has helped to popularize this procedure in
mainstream society. The American Society for Aesthetic Plastic Surgery
reports a large increase in the amount of people undergoing this
procedure.
Buttock Implants Options
There are various methods to augment the buttocks. The transfer of fat
to the buttocks that has been suctioned from areas such as the abdomen
and back can produce an increase in buttock projection along with an
improvement in contour of the suctioned areas. One drawback to the fat
transfer technique is that the results are not permanent since much of
the transplanted fat is reabsorbed during the first year. The injection
of permanent fillers into the buttocks such as silicone was quite common
15 years ago. This technique resulted in many cases of buttock deformity
including asymmetry, granulomas, nodularity, and migration of the
silicone. The insertion of a permanent implant into the buttocks
provides a predictable increase in size and projection.
Surgical Technique of Buttock Implants
I have performed many buttock augmentations over the past several years
in both men and women with a very high satisfaction rate. The surgery is
performed as an outpatient in my private surgical facility under general
anesthesia. The template of the implant is drawn on the buttocks to
guide the dissection. An incision is made in the midline between the two
buttocks. A pocket is made within the gluteus maximus muscle or between
the muscle and its envelope or fascia. In the U.S. only FDA approved
solid silicone implants can be used to avoid the risk of leakage
associated with gel implants. The implants are available in either round
or oval shape with various thicknesses. The implant shell can be either
smooth or textured. I prefer a round, smooth implant for the majority of
my patients. After the implant pocket has been created the buttock
implant is then inserted. Drains are often used and kept in place for
several days to remove any fluid that may accumulate around the
implants. Patients are usually able to walk out of the surgical facility
and are instructed to rest for several days. Walking is encouraged as it
has been found to decrease postoperative muscle spasms. Prophylactic
antibiotics are prescribed. Oral analgesics are usually needed for at
least one week. The drains are usually removed in 7-10 days when only
minimal fluid is present over a 24 hour period.
Results of the Buttock Implants
The results will be apparent immediately. In patients where the buttock
implant is placed within the muscle it may take a few months for the
implants to “settle” as the muscle relaxes. The scar usually heals very
nicely leaving only a thin line just above between the buttock crease
which would not be evident in even the smallest underwear or thong.
Normal exercise can be resumed for most patients in 4-6 weeks.
The best candidates for buttock augmentation with solid silicone
implants are men and woman who desire a fuller, rounder and more
projecting buttock. In patients who have experienced overall weight loss
or a loss of volume in the buttocks only, insertion of an implant can be
beneficial. Combining liposuction of the lower back and thighs with
buttock augmentation can enhance the shape of the body and yield
dramatic results.
The satisfaction rate of patients undergoing buttock augmentation has
been very high. I advise those patients interested in buttock
augmentation with a buttock implant to be realistic about their goals
and be committed to undergoing an elective surgical procedure that while
rewarding does carry some risk. People interested in more information
